Your First Appointment Today is Monday, September 06, 2010
Your first consultation is the most important. With a correct understanding of your condition we can
work to restore health to your systems. We need to work together to ensure that Stephen has a full picture of your health and lifestyle.
Please bring the following:
1: Most recent blood test,
Xrays CTs MRIs and any other tests performed to date.
2. Try to arrange to arrive
10 minutes early for your appointment or download, print and fill out the new
patient intake form.
3. Avoid drinking alcohol or
coffee before coming and it is best if you do not engage in heavy exercise that
day. We would prefer you arrive 5 minutes late than rush to get there on time.
4. It is not
advisable to have treatment with a overly full or empty stomach, limit yourself
to light meals but do not arrive hungry.
Every attempt is made to ensure that you will be treated
at your allotted time. This includes finishing on time even if a patient is
running late. From time to time in order to attend to seriously ill patients,
Stephen will go over time and apologises in advance for this.
His intention is to always
do his best to keep on time.
